Join us

Membership at Crocus is mandatory to participate in various programming and the Transitional Employment program. To become a member you must:

  1. Be 18 years of age or older.
  2. Have a diagnosed mental illness.
  3. Provide proof of diagnosis from a health professional.
  4. Complete an intake interview with a Crocus staff person.
  5. Pay a $1.00 lifetime membership fee.

Proof of diagnosis requires a completed referral form or a note verifying a diagnosed mental illness from a health professional. Proof of diagnosis forms can be delivered to Crocus either by fax or email from the health professional.
